Your written request must include the following information:
Your full name, address and contact information.
The type of legal claim you are pursuing.
The date and location of the incident.
A detailed description of the injuries you suffered.
Whether medical attention was required, and if so, the type and location of all medical care received.
For Inmate:
A federal law requires the completion of the institutional grievance process before a lawsuit can be filed in federal court. Therefore, you MUST include COPIES of the following documents:
The initial grievance filed.
All official responses and appeals.
DO NOT send original documents. The Department of Correction’'s policy prevents us from returning them to you.
Once we receive ALL of the requested information, we will formally evaluate your claim and notify you within 45 days if we decide we are able to investigate your claim.
